Initial Recognition Application
This application is a typeable PDF. Applicants have the option of saving the application to their own computer, typing in all of the required information, then printing the application, signing where appropriate, and mailing it. Alternately, all applications may be printed out and then filled in by hand in blue or black ink before mailing.

The first step in applying for portfolio recognition is to download the application. Read over it carefully to determine whether you are eligible. If you are, please fill out the application and return it to ANCC. IAFN members receive a discounted rate.

The second step in applying for portfolio recognition is to input your information into Decision Critical's portfolio, available at www.decisioncritical.com. ANCC has contracted this portfolio company to provide long-term storage of portfolio information to be used in subsequent renewals of your portfolio recognition. ANCC can not accept other on-line portfolios.

Renewal Application
Certifications must be renewed every 5 years. Renewal applications are due at least 3 months before your certification expiration date.
